General Information

	We will call your insurance company and verify your benefits and your out of pocket expenses.
	You will have a consult appointment to discuss all results of testing and establish treatment plan. At this time, tell your doctor several different dates that you are available for surgery. She will give your chart to our surgery scheduler.
	Contact the surgery scheduler to confirm you are able to proceed with surgery. Our surgery coordinator Mirasol can be reached at (972) 772-3234.
	It may take the surgery coordinator 1 week to coordinate requested dates with hospital, assistant surgeons, and contact insurance company for verification and pre-certification.
	The surgery coordinator will contact you with surgery time and date, location, and out of pocket expenses for proposed surgery.
	You will be scheduled for a pre op appointment at our office 2-5 days before your surgery. you will also pre op at the hospital the same day.
	Fees to our practice are due 72 hours prior to your surgery time. You may pay with cash, check or credit card.
	You will have had all required clinical testing ordered.

Pre Op Instructions

	Nothing to eat or drink after midnight the night before your surgery
	No water, candy or smoking on the morning of your surgery ( you may be cancelled by anesthesia if you eat, drink, or have candy the morning of your surgery)
	Avoid aspirin, and ibuprofen (Motrin) 2 weeks prior to your surgery
	Additional pre-op instructions will be given based on your surgical procedure
	Report to the hospital at your designated surgery time
